VAHS is a not-for-profit organization that has served the Downtown Eastside (DTES) for over 30 years. As an Indigenous-led and culturally grounded agency, we offer services by and for urban Indigenous people to promote physical, mental, emotional, and spiritual wellness.

Position Summary

We are seeking a compassionate and skilled Social Worker to join our Primary Care Clinic. This role provides direct, holistic care to patients while supporting wellness through advocacy, collaboration, and culturally safe practices.

Key responsibilities include:

  • Developing, implementing, and evaluating patient-centered care plans

  • Providing outreach services when needed

  • Coordinating with multidisciplinary teams and documenting case management

  • Advocating for patients navigating health and social service systems

This is an on-site role with regular hours of 8:30 AM–4:30 PM, Monday to Friday, with occasional flexibility to support evening programs.


Qualifications

Required:

  • Current full registration with the BC College of Social Workers

  • Master of Social Work + 1 year experience, or

  • Bachelor of Social Work + 3 years experience

  • A combination of education and experience may be considered

Knowledge, Skills, and Abilities:

  • Patient-centered, trauma-informed, and harm reduction care experience

  • Familiarity with applicable Acts and legislation (e.g., Mental Health Act, Human Rights Act)

  • Experience working with urban Indigenous populations and in culturally safe ways

  • Strong understanding of DTES-specific social and health systems

  • Crisis intervention, mental status, and suicide risk assessment skills

  • Ability to work independently and as part of a multidisciplinary team

  • Proficiency with electronic documentation systems and computer programs


Preferred Qualities

We are looking for someone with a deep understanding of Indigenous cultures and traditions. Ideal candidates will bring experience working with marginalized populations facing complex barriers such as trauma, poverty, substance use, and chronic illness.
